The rapid expansion of the field of ophthalmology over the next few decades is dramatically reflected in Yanoff and Duker’s Ophthalmology, 4th Edition, 2014, which is the subject of the present review.
Indeed, Lead Editor Myron Yanoff was one of the Contributors to Scheie and Albert’s 1977 Textbook.
"This latest attempt to cover the field in a single volume, addressed to the needs of ‘‘trainees, non-ophthalmologists and those general ophthalmologists (and perhaps specialists) who need an update [on topics] in which they are not expert,’’ is a folio-sized volume of 1404 pages, weighing in at just over ten pounds (4.5 kg), retailing for US$384.99. Detailed references and other material have been offloaded to the online version, saving about 100 pages in the hard copy! The Lead Editors have enlisted the contributions of 12 Section Editors who in turn have incorporated material from some 400 contributors. While the overall utility of such a tome for the intended audience is a matter for another time, this review will emphasize the treatment of clinical electrophysiology of vision and assess the suitability of the book as a resource to which one might refer interested students, residents or colleagues.
Electrophysiology is accorded a brief chapter (6.8) by Darin R. Goldman and Elisa Reichel in the ‘‘Ancillary Tests’’ portion of the Section devoted to Retina and Vitreous. A list of bullet points emphasizes the importance of ERG, multifocal ERG, EOG and the role of the International Society for Clinical Electrophysiology of Vision (ISCEV) in establishing standardized testing protocols. The text opens with a brief historical review beginning with Holmgren (1865), Dewar (1877) and Riggs (1941).
The full-field ERG is described in detail as set out in the current ISCEV standard (2008 update), though the terms ‘‘photopic’’ and ‘‘scotopic’’ are used in place of the recommended ‘‘light-adapted’’ and ‘‘darkadapted’’ in the standard. Normal waveforms for the five standard ERG responses are illustrated. Interpretation of abnormal responses is only briefly mentioned, with a more or less flat ERG from a patient with advanced retinitis pigmentosa depicted as the only example.
In summary, the role of clinical electrophysiology of vision in clinical practice is better documented in Yanoff and Duker’s Ophthalmology, 4th Edition than in the introductory textbooks of earlier generations.
However, the treatment is sparse, and this work will be of limited value as even a preliminary reference to thesubject for interested students or residents".
